If we are into latinising forenames, is not Theodoricus the proper form?

Only if assuming Terry is short for Terence! If it is a name in its own right, it is cognatye with French Thierry which is from Theodoricus not Terentius, puto te inventurum esse

Depends what sources, maybe. The people who write books about first names are not bound by verifiability constraints, so they are not reliable sources (I think). I would have said we need evidence that someone before us, writing Latin, has converted Terry to Theodoricus. Otherwise, he should stay at Terry.
